RATIONALE, AIMS AND OBJECTIVES: This paper deals with the problem of surgical waiting lists and is aimed, in particular, at comparing two different prioritization approaches: (1) the clinical assessment of treatment urgency aimed at categorizing patients into urgency-related groups (URGs) with a given recommended maximum waiting time for treatment; and (2) the implementation of an original prioritization scoring algorithm aimed at determining the relative priority of each patient in the waiting list and the corresponding order of admission. METHODS: A modelling (...) exercise based on a cohort of 236 patients enrolled on the waiting list of a surgical department in an Italian public university hospital, from 1 January to 30 June 2004, is presented. The comparison is based on a measure called need-adjusted-waiting-days, which allows to take into proper account both urgency and priority. RESULTS: The results show that both methods should be implemented simultaneously for increasing the department's performance in terms of both efficiency--outcome gained from a given amount of resources--and equity--how patients are admitted according to their need. CONCLUSIONS: Waiting list prioritization should not be limited to classifying patients into URGs, but to using a scoring system as well, in order to schedule patient admissions in an explicit and transparent way. This paper presents and discusses a novel approach to indeterministic belief revision. An indeterministic belief revision operator assumes that, when an agent is confronted with a new piece of information, it can revise its belief sets in more than one way. We define a rational agent not only in terms of what it believes but also of what it desires and wants to achieve. Hence, we propose that the agent’s goals play a role in the choice of (possibly) one of (...) the several available revision options. Background The use of lengthy, detailed, and complex informed consent forms is of paramount concern in biomedical research as it may not truly promote the rights and interests of research participants. The extent of information in ICFs has been the subject of debates for decades; however, no clear guidance is given. Thus, the objective of this study was to determine the perspectives of research participants about the type and extent of information they need when they are invited to participate in (...) biomedical research. Methods This multi-center, cross-sectional, descriptive survey was conducted at 54 study sites in seven Asia-Pacific countries. A modified Likert-scale questionnaire was used to determine the importance of each element in the ICF among research participants of a biomedical study, with an anchored rating scale from 1 to 5. Results Of the 2484 questionnaires distributed, 2113 were returned. The majority of respondents considered most elements required in the ICF to be ‘moderately important’ to ‘very important’ for their decision making. Major foreseeable risk, direct benefit, and common adverse effects of the intervention were considered to be of most concerned elements in the ICF. Conclusions Research participants would like to be informed of the ICF elements required by ethical guidelines and regulations; however, the importance of each element varied, e.g., risk and benefit associated with research participants were considered to be more important than the general nature or technical details of research. Using a participant-oriented approach by providing more details of the participant-interested elements while avoiding unnecessarily lengthy details of other less important elements would enhance the quality of the ICF. In a recent paper [J. G. Vargas and D. G. Torr, Found. Phys. 27, 599 (1997)], we have shown that a subset of the differential invariants that define teleparallel connections in spacetime generates a teleparallel Kaluza-Klein space (KKS) endowed with a very rich Clifford structure. A canonical Dirac equation hidden in this structure might be uncovered with the help of a teleparallel Kähler calculus in KKS. To bridge the gap to such a calculus from the existing Riemannian Kähler calculus in (...) spacetime, we commence the construction of a teleparallel Kähler calculus in spacetime. In the process, we notice: (a) Unknown to him, one of Einstein's equations in his attempt at unification with teleparallelism states that the interior covariant derivative of the torsion is zero. (b) A mechanism exists in the tangent bundle of teleparallel spaces for producing confinement (in the applicable cases, one would have to show why nonconfinement also occurs, rather than the other way around). (c) When the torsion is not zero, the interior covariant derivative in the sense of Kähler, δF, does not coincide with *d*F. The system (dF = 0, δF = j) rather than (dF = 0, *d*F = j) should then be used for generalizations of Maxwell's electrodynamics. (shrink)

The current times call for reforms in educational processes. The Covid-19 pandemic had an unforeseen impact on the educational system in all countries. This need for change requires new pedagogies and new methods for teaching and learning. Understanding the need for change is essential for the formulation of adaptive proposals, as well as for the generation of training activities to complement the teaching curriculum. New educational practices lead to a vision of educational quality, with new approaches that allow the continuous (...) integration of knowledge and permanent interaction with the student. This paper presents an analysis of the new teaching methodologies in times of confinement due to the pandemic caused by Covid-19. SummaryIntense, sustained nursing lengthens inter-birth intervals and is causally linked with low natural fertility. However, in traditional settings, the effects of such nursing on fertility are difficult to disentangle from those of nutrition. Results from an prospective, direct observational study of reproductive function in well-nourished Amele women who nurse intensively and persistently but who also have high fertility are here presented. Endocrine measures show that ovarian activity resumes by median 11·0 months postpartum. Median duration of postpartum amenorrhoea is 11·3 months, (...) time to next conception is 19·0 months, and the inter-birth interval is 28·0 months. Average life time fertility is 6·8. High fertility in Amele women is due both to refractoriness of reproductive function to suckling stimuli, and to maintenance of equivalent age-specific fertility rates across the reproductive life span. Bazzocchi disposes the text of the Tractatus in a user-friendly manner, exactly as Wittgenstein's decimals advise. This discloses the logical form of the book by distinct reading units, linked into a fashioned hierarchical tree. The text becomes much clearer and every reader can enjoy, finally, its formal and literary qualities.

This 1992 book examines alternative methods for achieving optimality without all the apparatus of economic planning, or a vain reliance on sufficiently 'perfect' competition. All rely entirely on the self-interest of economic agents and voluntary contract. The author considers methods involving feedback iterative controls which require the prior selection of a 'criterion function', but no prior calculation of optimal quantities. The target is adjusted as the results for each step become data for the criterion function. Implementation is built in by (...) the incentive structure, and all controls rely on consistency with the self-interest of individuals. The applicability of all the methods is shown to be independent of the form of ownership of enterprises: examples are given for industries which are wholly privately owned, wholly nationalized, mixed and labour-managed. According to the prevailing sentiment, changing the past is logically impossible. The prevailing sentiment is wrong. In this paper, I argue that the claim that changing the past entails a contradiction ultimately rests upon an empirical assumption, and so the conclusion that changing the past is logically impossible is to be resisted. I then present and discuss a model of time which drops the empirical assumption and coherently models changing the past. Finally, I defend the model, and changing the past, (...) against objections. Some philosophers argue that any attempt to model changing the past will either be contradictory or really model avoiding the past. Using Nicholas Smith's (1997) argument as a basis, I formulate a generic version of this Avoidance Argument. I argue that the Avoidance Argument fails because (i) it involves an equivocation of what is meant by ‘bifurcation of the time of an event’ and (ii) resolving the equivocation results in the falsity of at least one of the premises. The method of interpreting political theory in relation to its specific historical contexts offers particular insight into the work of Karl Marx. When pre-capitalist societies are understood in relation to Marx's rigorously conceived capitalist mode of production, it is apparent that the context in which Marx produced his very earliest work was itself pre-capitalist. It can then be recognized that Marx began by making a significant contribution to an existing framework of critical political theory but also that, following a critical (...) confrontation with English political economy in 1844, he opened new avenues of thought in relation to a new context: capitalism. Quantum mechanics, which describes the behavior of subatomic particles, seems to challenge common sense. Waves behave like particles; particles behave like waves. You can tell where a particle is, but not how fast it is moving--or vice versa. An electron faced with two tiny holes will travel through both at the same time, rather than one or the other.
